Site Specific Information Form (SSIF)

Please submit the Site Specific Information Form (SSIF) of the standard application with a signed copy of the Principal Investigator's CV.

Please note that the SSIF is largely pre-populated from the main application, however some additional information should be completed by the Principal Investigator.

When completed, telephone ORECNI for a Reference Number, and then lock the online form, print and send the signed hard copy with the other required documents to ORECNI, Office Suite 3, 1-4 Haslem's Lane, Lisburn, BT28 1TW.

Please note that if you have declared your study to be Site Specific Exempt at A6 of the main application, you do not need to complete the SSIF section of the form or send any part of this to ORECNI.

The SSIF section is generated to allow researchers outside NI to apply for R&D approval. R&D in Northern Ireland is managed through the appropriate Trust Research Governance Pathways prior to ethical review.
